How it works
Claims may be reported by agent or the insured. Notice of each incident, claim or suit should be immediately reported to Raphael & Associates. Raphael & Associates provides our clients with a quality service they can trust.
Steps to take to file a claim
1. Get in touch. Choose a contact method you are most comfortable with.
2. Collect information. When reporting a claim, please provide a completed Loss Acord form. At a minimum, please include the following on the Acord form:
- Reference Timby Claims
- Policy Number
- Insured contact information: name, phone number, address, email
- Agent contact information: name, phone number, address, email
- A detailed description of the loss
Please refer to your policy for the complete list of your duties in the event of a claim.
